"Advanced practice nurses can provide the primary and preventive care... to reduce waste and bring down costs while increasing access and quality." - AARP

How do I qualify for this service?

If you are unable to leave your home without the help of another individual or assistive device (walker, wheelchair, etc.), or if leaving your home is physically taxing, you are an ideal candidate for this unique home visit program.

Will my insurance cover this service?

If you qualify, traditional Medicare policies will cover home visits just like a trip to a primary care office. We accept Medicare, Medicaid and secondary insurance. You will not be asked for payment at the time of your visit. We will bill your insurance carrier on your behalf.

Can a Nurse Practitioner be my primary care provider?

Yes. A Nurse Practitioner is an independent provider that may diagnose, prescribe medications, order tests and treat patients according to Kentucky State Law. The NP model has been gaining popularity for its patient centered approach to care, and many patients prefer the NP model as their choice for delivering primary care.

Why should I choose a Nurse Practitioner?

Many communities across the country rely solely on Nurse Practitioners for their care. NPs provide high-quality, cost-effective personalized healthcare. They are able to treat acute illness, coordinate care with multiple providers, manage chronic conditions and educate patients on maintaining a healthy lifestyle.

How do I schedule a home visit?

Please call our office to schedule an appointment and one of our providers will be out to visit with you or your family member at home.
